At Midland Oral & Maxillofacial Surgery PA we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. Fee Transparency: Nothing is more maddening than getting nickel and dimed and being allowed to proceed into something without a declaration of the cost involved. Dr. Gray will make every effort to show you written estimates and to stick with those estimates throughout your case. Expect a verbal and or written explanation about a procedure and your cost prior to starting anything. Expect no underhanded increases in what you owe or "extra" items that you were not told about. If we quote you for a surgery we will honor that estimate for 6 months. That said Please understand that there are times that what is required to achieve your result cannot be accomplished as thought previously. In these rare cases expect another discussion, all your questions answered and again an estimate that will be treated as above. 

Dental/ Medical Insurance:

Dr. Gray accepts consignment of ANY dental insurance. When you come for a consultation you will be given a written estimate for what your specific insurance company is wiling to cover. After a procedure is completed this is then filed with your insurance company who will then decide what they will or will not pay. Unfortunately even a pre-determination will not obligate your insurance company to cover your surgery as they estimated. Our goal with these estimates is to be as close as possible to the actual amount that they will cover but this cannot always be predicted 100%. We understand that when treatment estimate and actual treatment vary wildly that this can affect ones decision to proceed with treatment or to not do so. Our goal is to error on the side of owing you a refund rather than you owing more money for a procedure.

 

Notice:Because insurance carriers have many plans and who is covered changes, please call your provider to assure "provider" status if that is desired. 

As of (5-24-16) Dr. Gray is Dental a Provider for:  

Aetna, Assurant, Guardian, Maverest, Principal, United Concordia

We expect further affiliation to become effective in next 3 months with: Ameritas, Cigna DPPO, Dental Network of America, Metlife PDP

 

Financial Arrangements:

We Accept Cash, Check, Visa, Mastercard and In house financing via CareCredit (12-month no interest)
